Position Summary

ABOUT THE ROLE:

The Health Independence Program (HIP) at Werribee Mercy Hospital specialises in best practice in-centre and home based programs to help clients living within the Wyndham catchment improve their health and achieve their rehabilitation goals. HIP consists of five services, including Complex Care, Community Based Rehabilitation, specialist clinics: Falls and Balance and Continence and Residential InReach.

In this role you will support the Health Independence Program team in managing clients at nutritional risk by completing nutrition assessments and providing interventions to community based clients for a wide range of chronic health conditions. The ideal candidate will have a strong work ethic and have the confidence to manage a busy and growing workload

 

SUMMARY OF THE ROLE:

  • 45.6hrs (0.6EFT), permanent part time 
  • Located at Werribee Mercy Hospital/Health Independence Program (HIP)
  • Assessment and managment of nutrinional needs of clients attending HIP
  • Sound Knowledge of primary health care issues
  • Working collaboratively with the team to promote a culture of client centred care with continuous feedback
  • Foster an environment which encourages relationships and communication between external organisations, within the team and with client/ carer

ABOUT YOU:

  • Bachelor of Nutrition and Dietetics, Master of Dietetics or equivalent qualification.
  • Eligible for membership to the Dietitians Association of Australia and APD status.
  • Current Full Victorian (or equivalent) Drivers Licence
  • Strong, relevant clinical skills and 2 years’ experience in sub-acute/ community setting
  • Previous experience in supervising Dietetic students (Desirable)
